That's interesting, I don't recognise the description from sfl's dictionary description. I recognise Cambridge's descriptions though:
to look at someone in an unpleasant or offensive way:
He was leering at her.
(especially of men) to look at someone in a sexually interested way:
He was always leering at female members of staff.
(The noun is the facial expression described by the verb)

I basically understand a leer to be a look someone else gives you that makes you feel uncomfortable or disgusted because of what they look like they might be thinking about in connection with you (often but not always a sexual intention).

Sure, but a side glance or roll of the eye are not common features in my experience. People nowadays tend to look at you directly when leering. I don't know if I'd even recognise a side glance or a roll of the eye as a leer.

The smile that Uriah Heep makes right before he gets slapped would be the kind of "leer" mentioned in the book you've quoted from.

While the definition "to look at someone in an unpleasant or offensive way" is totally correct, "leering" in modern speech generally refers to men looking at women in an obviously lustful manner that is generally unwanted. These cartoons show this type of leering:
